TeleHealth

TeleHealth is a Provincial network of secure videoconferencing facilities, connecting health providers and patients. Patients have the option of scheduling live consultations with participating TeleHealth specialists, using audio visual equipment that allows both parties to see and hear each other from remote locations. For many patients, this option eliminates the need for long journeys to attend consultations that do not require physical examination. In some cases, TeleHealth can reduce wait time for consultations. Please discuss with your specialist if you are interested in registering for this new consultation method.

Travel Assistance Program

The TAP helps reduce travel costs for eligible Cortes residents who must travel within the Province for non-emergency medical specialist services. To be eligible for this program you must:

You must be a BC resident and enrolled in MSP, and must have a referral from a physician or nurse practitioner to a specialist or for specialty services.

The staff at the Cortes Health Centre will provide the TAP forms to eligible patients on behalf of the referring physician, and clarify eligibility.

Augmented Home Support Program (AHSP)

CCHA has operated the Augmented Home Support Program (AHSP) since 2014. The goal of the program is to support Cortes residents to be able to stay in their homes while they age or recover from hospitalizations, illness, surgeries and at the end-of-life.

Clients are Cortes resident of any age e.g. seniors, those with chronic health challenges or those experiencing disability after illness, injury, surgery or hospitalization. We do not provide any hands-on personal care or therapy.

AHSP provides up to 6 hours per week for:

  • Support with light housekeeping, laundry and bed linen changes etc
  • On Island transportation to the Health Centre. Pick up from pharmacy, post office and grocery shopping.
  • Firewood and water can be moved to an accessible spot.
  • Help with garbage/recycling for roadside pickup
  • A hospital bed can be set up and dismantled.
  • The workers provide safety checks and socialization while they work
  • Limited meal support may be available

If you, or someone you know, could benefit from our help please call Bernice at 250 935 6405 or email ccha.programs@gmail.com for more information.  If you have a planned surgery coming up, please contact us ahead if you think you will need our help on returning home.  Funding is through the generous support of Island Health and donations. The program is not directly affiliated with the Cortes Health Centre medical practice and service is confidential.
